The Two Main Paths in Medicare
When you become eligible for Medicare, you face one fundamental choice. Both paths have distinct advantages depending on your doctors, travel lifestyle, and budget.
Original Medicare + Medigap
Parts A & B + Medicare Supplement (Plan G or N) + Standalone Part D
- Any Doctor Nationwide: Visit any physician, specialist, or hospital in the US that accepts Medicare without network restrictions.
- No Referrals Required: See specialists directly without waiting for insurance pre-authorizations.
- Predictable Out-of-Pocket Costs: Supplemental insurance pays the deductibles and coinsurance that Original Medicare leaves behind.
- Ideal for Travelers: Coverage travels with you anywhere across the country.
- Separate Drug Plan: Requires adding a standalone Medicare Part D prescription drug plan.
Medicare Advantage (Part C)
Bundled hospital, medical, and prescription drug coverage
- All-in-One Simplicity: Combines Part A (Hospital), Part B (Doctor), and usually Part D (Prescriptions) into a single plan card.
- Low or $0 Monthly Premiums: Many plans offer $0 monthly plan premiums (you continue paying your standard Part B premium).
- Extra Benefits Included: Often includes routine dental exams, vision eyewear, hearing aids, and fitness gym memberships.
- Provider Networks: Care is coordinated through designated HMO or PPO networks of doctors and hospitals.
- Annual Out-of-Pocket Maximum: Caps your financial exposure on covered medical services each year.
Medicare Enrollment Timelines You Must Know
Missing an enrollment window can trigger lifetime late enrollment penalties on Part B and Part D. Here is when you can take action:
Initial Enrollment Period (IEP)
A 7-month window: 3 months before your 65th birthday month, your birth month, and 3 months after. This is your primary window to enroll without penalty.
Annual Election Period (AEP)
The yearly open window for everyone on Medicare. Switch from Original Medicare to Advantage, change Advantage plans, or modify your Part D drug plan. Changes take effect January 1.
Advantage Open Enrollment (OEP)
If you are currently enrolled in a Medicare Advantage plan, you can make a one-time switch to another Advantage plan or return to Original Medicare during the first quarter.
Special Enrollment Period (SEP)
If you or your spouse continue working and have qualifying creditable employer group health coverage, you may delay Part B without penalty until retirement.
Big News for Part D: The $2,000 Out-of-Pocket Prescription Cap
Under recent federal healthcare law, Medicare Part D out-of-pocket prescription drug costs are contractually capped at $2,000 per year, and the dreaded "donut hole" coverage gap has been completely eliminated. Additionally, the Medicare Prescription Payment Plan allows seniors to opt-in to smooth their drug costs over equal monthly installments rather than paying high deductibles upfront in January. Mary Lee will review your exact medications to verify which plan saves you the most.
